COVID-19 SELF-ASSESSMENT

This self-assessment tool, developed by Ministry of Health Malaysia partnership with DoctorOnCall, will help determine whether you may need further assessment or testing for COVID-19

This guideline is issued to provide measures in facing the current health situation. The objective is to minimize the contact between students and lecturers as to prevent the outspread of Covid-19. Outside of T&L, all staff and students must adhere to any preventive measures issued by the Ministry of Health.

Please make sure that any information received is verified with valid sources and this comes from UTHM official source which is the Office of Marketing & Corporate Communication.

Fill-up the form

All those students who attended the recent “Jord Qudama and Ulama Malaysia” at Masjid Jamek Sri Petaling to fill-up this form and have themselves checked at the nearest clinics if they are displaying symptoms of the virus.
